This form is for the invited artists featured on our Small Stuff shelves. It creates the database for the point of sale and to reduce errors at checkout. 

Please make sure we can match the item on the shelf with the item in the database. If your artwork has individual titles, please bring it in labeled consistently. You may also just number your pieces and add the corresponding number in 'Title or description' field.
